These forms may be found on the Chiles Athletic Website.

  1. Activity Participation
  2. Physical Form
  3. Release Liability
  4. HIPPA

Varsity Tryouts:   Monday, January 31st thru Thursday February 3rd – 3:00-5:15 pm
Full equipment required

First Regular Season Practice:  Monday, January 31st

Regular Season Practice Time:  Monday – Friday 3:00-5:15.  Players must be picked up by 5:30.

All players must have all equipment before they may practice during the regular season. Required equipment:

  • Chiles Helmet – gray or silver
  • Shoulder Pads
  • Arm Guards
  • Black Gloves
  • Mouth Guard
  • Protective Cup
  • All White Stick Head and Pocket
  • Silver/Alloy Shaft
  • Black Cleats
