All players MUST have the following FOUR required forms completed and turned into Coach Sparrazza before they may tryout. These forms may be found on the Chiles Athletic Website.

- Release Liability

All players must have all equipment before they may practice during the regular season. Required equipment:
- Chiles Helmet – gray or silver
- Shoulder Pads
- Arm Guards
- Black Gloves
- Mouth Guard
- Protective Cup
- All White Stick Head and Pocket
- Silver/Alloy Shaft
- Black Cleats
